If you are an outpatient provider referring a patient for an appointment, please fax your referral and we will contact your patient.

If you are an inpatient provider referring a hospitalized patient, please fax your referral, a copy of the admission note, and a copy of the discharge narrative (or physician progress note from the day of discharge) and we will contact you. We may require your patient to be present at the time of scheduling to ensure patient consent. Referrals for patients who have required inpatient hospitalization more than once in the previous twelve months, or who have required a hospitalization of more than 2 weeks, will be accepted on a case-by-case basis.
